Clinical judgment

American medicine runs on protocols, guidelines and pathways — and every one of them carries language reserving the clinician’s judgment to depart from it. The guideline states what is usually right. The physician decides whether this is a usual case.

COBRA

American health insurance is usually tied to the job, and federal law allows a departing employee to continue the employer’s plan — at the full unsubsidised cost, which is typically several times what they were paying. The bridge exists and it is expensive.

The prescribed cure

A German physician can prescribe a Kur — a multi-week stay at a spa or rehabilitation clinic, substantially funded by the health or pension insurance. Exhaustion is treated as a medical condition with an institutional remedy, and the time away does not come out of holiday entitlement.

The Working Hours Act

Das Arbeitszeitgesetz. German law caps the working day at eight hours, extendable to ten only if the average returns to eight within six months. The ten-hour limit is absolute. Eleven uninterrupted hours of rest are required between working days. Breaks are mandatory: thirty minutes after six hours of work, forty-five after nine.

Since a Federal Labour Court ruling in 2022, all working time must be recorded.

The yellow booklet

Das gelbe Heft. A German child receives ten medical examinations in the first six years, U1 through U9 including U7a, at fixed intervals from the minutes after birth to just before school.

They are a statutory insurance benefit under § 26 SGB V, and the content and timing are set nationally by the Gemeinsame Bundesausschuss. Each is recorded in the yellow booklet the parents receive at birth, which carries the schedule for every appointment that follows.

In Bavaria, Hesse and Baden-Württemberg, attendance is legally required.

Burnout

The World Health Organization classified burnout as an occupational phenomenon in 2019, and American surveys report it at high rates across professions. It is discussed in the United States as a condition of the individual, with individual remedies — resilience, boundaries, self-care.

The household stockpile

Vorratshaltung. The German Federal Office of Civil Protection publishes a recommendation that households keep supplies of food and water for a period of days, with a checklist of what and how much. The state issues a stockpiling guide to ordinary citizens as standing advice, not in response to any particular threat.

Evidence-based practice

American medicine reorganised itself around the phrase in the 1990s, and it spread to nursing, education, social work and policy. The argument was that established practice rests on authority and habit, and should rest on outcomes measured in trials. A whole profession accepted that its procedures had the wrong derivation.

The specialist physician

Becoming a Facharzt takes five to six years of full-time hospital work, following six years of basic medical training.

Each state’s Weiterbildungsordnung specifies the minimum content and duration for every specialty. All training periods must be recorded in a logbook and confirmed by the supervising trainer, after which the doctor registers for the specialist examination before the state medical association.

Eleven or twelve years before the title. The German patient asking for a Facharzt is asking for that, and knows it.
